Swan Hill & the Mallee

Growing up in Swan Hill, we know it is not always easy to get access to specialised care.  We are excited to offer a home visiting service to the Mallee region monthly to support you and your needs.

This may include specific assessment and treatment of your progressive neurological condition, such as Multiple Sclerosis, Parkinson's, Post Stroke, and other progressive conditions.

We offer both in-home and telehealth services to help you keep moving.  This includes NDIS Assessment & Report Writing,  assistance with applications for the NDIS & collaborating with other members of your team to help you maintain your independence and community engagement.
